Job Description

• Provide physical therapy assistant services in a home health setting.
• Conduct home visits and periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (some positions may have alternate schedules such as 4x10 shifts).
• Patient visits include both care facilities and patient homes.
• Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
• Local candidates accepted.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience (recent experience required; first-time home health workers not accepted).
• Current Physical Therapy Assistant (PTA) license in hand (pending licenses accepted only if they can be obtained within a few days).
• CPR and BLS certification required.
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.
• Must be able to meet productivity expectations (~25-30 weekly units or 32 points weekly).
• Experience with HomeCare HomeBase, Oasis, and Outlook EMR systems preferred.
• On-call requirements vary; discuss during interview.
• Weekend requirements vary by location and profession; discuss during interview.

Additional Requirements & Compliance

• Must complete standard background checks including:

  • 7-year work and residence history verification.
  • Sex offender and nationwide criminal background checks.
  • State-specific background checks as applicable.
  • Drug screening (10-panel).
• Health documents required:

  • Physical exam within 12 months prior to start.
  • Negative TB test or chest X-ray within 12 months.
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
  • Flu vaccination or documented declination.
  • COVID-19 vaccination series or declination.
• Must complete facility-specific compliance training and review policies prior to start.
• Must have personal or auto insurance policy (for home health placements).
• References: Two professional references required.
• Work history verification for at least the last 7 years or two most recent employers.

Travel Experience

• Prior travel experience is not explicitly required, but recent home health experience is mandatory.
