Physical Therapist - Pediatrics (Travel/Contract)

Job Description

• Position at a Pediatric Pain Rehabilitation Center
• Requires experience with chronic pain, rehabilitation or habilitation model of care
• Pediatric experience required

Schedule

• Day shift
• 5 days per week, 8 hours per day (7:45 AM – 4:45 PM)
• 40 hours per week

Required Qualifications

• Massachusetts Physical Therapist license required
• Experience: Minimum 1 year in specialty (Therapy/Rehabilitation with pediatric focus and chronic pain/rehabilitation experience)
• Proof of name change documentation if applicable
• Two professional references from two separate employers within the last 3 places of employment
• 10-year work history with explanations for gaps over 30 days

Certifications & Competencies

• Basic Life Support (BLS) certification (AHA or American Red Cross) required
• Advanced Cardiac Life Support (ACLS) required if assignment extends beyond 13 weeks (specific units only)
• Pediatric Advanced Life Support (PALS) may be required depending on unit
• Verification of professional certification must be completed within 30 days prior to start
• Medication Test/Pharmacology Competency Exam required for RNs and LPNs (not applicable for PT)

Background & Compliance Requirements

• 10-year criminal background check within 6 months prior to start, including aliases and multiple federal/state databases
• Massachusetts CORI release form required, must be run within 60 days of start date
• EEC Background Check Consent Form required for behavioral health/psych positions (not typically PT)
• EPLS/GSA/SAM and HHS/OIG checks required
• Annual background check updates required for direct candidates
• Drug screening (9 panel) within last 12 months (not required for all contract types)

Health & Immunization Requirements

• Physical exam within 1 year of start, signed by MD, DO, PA, NP, or registered DC, stating cleared to work with no restrictions
• Immunizations required prior to start:

  • Tetanus/Diphtheria/Tdap (Td not accepted)
  • Negative TB test or chest x-ray per facility protocol (annual requirement)
  • MMR (2 doses or positive titer)
  • Varicella (2 doses or positive titer)
  • Hepatitis B (series and/or declination or positive titer)
• Seasonal flu vaccine or approved exemption required annually (vaccines must be administered on or after 8/1)
• COVID-19 vaccine or approved exemption required; seasonal COVID vaccine preferred but not mandatory
• Respiratory Safety Agreement and clearance required; facility-specific fit testing completed onsite

• Onboarding packet to be completed approximately 2 weeks prior to start
• Various facility-specific forms and attestations required (e.g., data collection, flu exemption, COVID exemption)
• Candidates must comply with facility’s immunization and health documentation policies, including timely submission of required forms
