Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Directly supervise and evaluate Home Health Aides and other LVNs/LVNs.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, and Follow-up Visits.
• Must be willing to travel up to 75 miles from the home clinic; average travel about 100 miles per week.
• Use PointCare electronic medical record system for documentation.
• Proficiency with HomeCare HomeBase (HCHB) required; retraining provided if last use was over 6 months ago.
• Typical work schedule is Monday through Friday, 8:00 AM to 5:00 PM (5x8 hours).
• Some positions may have a 4x10 schedule (Friday-Monday), but weekend rotation and on-call requirements vary and should be discussed during the interview.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience in the specialty.
• Recent specialty experience required; first-time travelers are not accepted.
• Current, active LVN/LPN license for Oregon (license must be in hand; pending licenses are not accepted).
• CPR and Basic Life Support (BLS) certification required.
• Reliable transportation, current driver's license, and agency-required liability insurance required.
• Proficiency with HomeCare HomeBase (HCHB) EMR system.
• Must be able to meet productivity expectations of 30-32 points per week (points assigned based on visits and activities).

Additional Requirements and Compliance

• Must complete a standard background check including 7 years of history, sex offender search, and other state-specific screenings.
• Drug screening (10-panel) required.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 vaccination or declination.
• Flu vaccination or facility-specific declination documentation required.
• Must review and sign various compliance documents prior to start, including confidentiality, HIPAA, privacy policies, and home health-specific scenarios and policies.
• Personal or auto insurance policy required, with candidate listed as insured or driver.
• Two professional references required.
• Verification of work history for at least the last 7 years or two most recent employers.

Work Environment

• Dress code: Scrubs.
• Home health setting with travel between patient homes.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed).

Notes

• Weekend and on-call requirements vary by location and should be clarified during the interview.
• Local candidates are accepted under the same terms as travelers.
• No guaranteed hours; if patient census is low, options to float to nearby clinics may be discussed.
