Job Opportunity: Home Health/Hospice RN

Location: Iron Mountain, Michigan
Contract Length: 13 weeks

Job Summary

Seeking a Registered Nurse with recent Home Health (HH) experience to provide care for Home Health and Hospice patients. Patient cases include CHF, Diabetes, Renal Disease, Wound Care, IV therapy, and management of drains and other devices.

Key Responsibilities

• Provide care to approximately 6 patients per day (Admission plus 3-4 follow-ups)
• Perform Start of Care (SOC), Re-certifications, and Resumptions
• Manage wound care and infection control in the home environment
• Participate in on-call rotation (nights and weekends)
• Work both Home Health and Hospice patients as needed (primary hospice patients typically not assigned)
• Follow office scheduling: Monday-Friday 8:00 AM to 4:30 PM with rotating weekends and holidays

Required Qualifications

• Current RN license for Michigan
• BLS (AHA) certification required
• Recent Home Health experience within the last 2 years
• Proficiency in OASIS documentation
• Wound care experience
• Excellent infection control practices in home care settings

Work Schedule & Requirements

• 5 days per week, with rotating weekends (typically every 4th or 5th weekend)
• On-call rotation: usually one night per week and one weekend per month
• Up to two holidays per year required
• Dress code: Any solid color, clean and professional attire

Additional Information

• No ventilator support provided for patients in Home Health
• CNA support available for Home Health/Hospice aides
• Must clock in and out at the facility
• Prior experience with this healthcare system requires a minimum 6-month gap before reapplying

Experience Requirements

• Minimum 2 years of specialty experience in Home Health nursing
