Physical Therapist - Minnesota (Albert Lea)

Contract Length

  • 13 weeks

Job Description

  • Assess clients’ needs and develop therapy plans.
  • Administer and perform therapeutic interventions.
  • Maintain clients’ self-esteem, privacy, and confidentiality.
  • Provide clients opportunities for independence and choice consistent with care plans.
  • Notify physician of changes and process orders.
  • Document observations and services provided.
  • Provide direct consultation with physicians and interdisciplinary team members.
  • Provide training on programs, adaptive equipment, and other interventions.
  • Utilize and supervise therapy assistants as per Federal and State regulations.
  • Follow infection control and safety standards.
  • Comply with all related laws, regulations, and company policies.

Work Schedule

  • Monday through Friday or shifted schedules (Tuesday-Saturday, Sunday-Thursday, or four 10-hour days).
  • Guarantee: 36 hours per week for twelve-hour shifts, or 40 hours per week for eight to ten hour shifts.
  • Work week runs Sunday through Saturday.

Locations Covered

  • Skilled Nursing Facility (SNF) and Home Health coverage areas including Preston Home Health, Austin SNF, and occasional coverage for PTO at Albert Lea SNF.

Supervision

  • Will supervise 2 therapy assistants.

Training

  • New graduates will be trained; therapy supervisor and clinical specialists available onsite.

Software Used

  • Net Health and HCHB

Productivity Expectations

  • SNF: 82% productivity (evaluation, group, and concurrent time counted in full).
  • Home Health: 90% productivity including drive time and documentation.

Mileage and Drive Time

  • Paid between locations and from patient to patient in Home Health.

Required Qualifications

Education

  • Completion of an accredited physical therapy program.

Experience

  • Prior work experience as a therapist in a clinical setting is helpful.

Licensure & Certification

  • National registration and licensure for the state of practice required.
  • BLS Certification required.
  • Must be at least 18 years of age.

Skills and Abilities

  • Basic physical therapy skills.
  • Ability to communicate and comprehend effectively in English, both orally and in writing.

Physical/Mental Requirements

  • Ability to perform essential job functions including operating equipment, communicating, assessing, and maintaining physical positioning as needed.

Supervisory Responsibilities

  • Interviewing, hiring, training employees.
  • Planning, assigning, and directing work.
  • Appraising performance, rewarding, disciplining.
  • Addressing complaints and resolving problems.
  • Fostering employee engagement.

  • No on-call or call-back required.
