Approximately 6.5 patients treated per day
Responsibilities include start of care (SOC), recertifications (ROC), evaluations, and daily visits
Patient diagnoses commonly include CHF, orthopedic conditions, and pneumonia
Work setting: Home Health
No acute inpatient, acute rehab, skilled nursing, or outpatient involvement

2 weeks of orientation with a preceptor/work coach
Clinical manager or educator available 24/7 for emergencies and 8 AM - 5 PM for routine support
Phone and tablet provided for work
