Physical Therapist - Home Health (San Jose, CA)
Employment Type: Travel/Contract
Contract Length: 16 weeks (approximately 4 months)
Job Overview
- Provide physical therapy services primarily in home health settings, including wound care and geriatric patients.
- Minimum of 23-24 patient visits per week required to qualify for a 40-hour guarantee.
- Focus on quality patient care rather than number of patients per day.
- No weekend shifts required.
- Day shift: 8:00 AM to 5:00 PM (5 days a week, 8 hours per day).
- Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed.
- Mileage reimbursed from first to last patient of the day (not to/from home).
- Locals within a 50-mile radius are accepted.
- Dress code: Any color scrubs and closed-toe shoes.
- Free residential parking available.
Required Qualifications
- Valid Physical Therapist license in California.
- Minimum of 1 year of Physical Therapist experience, including experience within the last year.
- Experience in a Home Health setting.
- Basic Life Support (BLS) certification.
Compliance & Documentation Requirements
- All required documents must be submitted and approved before starting patient care; compliance processing may take 2-3 weeks.
- Required documents include but are not limited to:
- Government-issued photo ID (Driver’s License/State ID)
- Background checks (including SS trace, county searches, nationwide sex offender registry) within 1 year of start
- Annual federal exclusions and sanctions checks (EPLS/GSA/SAM, HHS/OIG)
- Certifications: BLS (required), ACLS and PALS preferred but not mandatory
- State license primary source verification
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases
- Immunizations/vaccinations or documented exemptions for: COVID-19, Influenza (flu), Tetanus/Diphtheria/Tdap, Measles, Mumps, Rubella, Hepatitis B, Varicella
- Negative TB test or chest X-ray within 1 year of start (with specific protocols for past positives)
- 9-panel drug screen within 1 year of start
- Emergency contact form
- Auto insurance card (if applicable)
- Skills checklist and competency exams as applicable
Additional Information
- Contract length for less experienced clinicians (less than 2 years total experience) is non-negotiably 6 months.
- Territory and county coverage details will be discussed during the interview; may include surrounding counties.
- Clinicians are expected to be punctual, patient-care focused, self-starters, and diligent in completing charting.
- COVID vaccination is required; religious exemptions allowed. Flu vaccination is encouraged but may be declined with proper documentation.
