Registered Nurse Travel/Contract Position

Location: Prestonsburg, Kentucky
Contract Length: 13 weeks

Required Qualifications

• Valid Registered Nurse (RN) license in Kentucky
• Minimum of 1 year of Med Surg / Telemetry experience within the last year
• Advanced Cardiac Life Support (ACLS) certification
• Basic Life Support (BLS) certification issued by the American Heart Association
• NIH Stroke Scale (NIHSS) Certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Experience

• Minimum years of experience: Not explicitly stated; standard RN experience expected

Health and Compliance Requirements

• Annual physical exam or physician’s note within 1 year prior to start, stating clearance to work without restrictions
• Fit mask test to be completed onsite; PAPRs not accommodated if facial hair present
• Valid Tetanus/Diphtheria/Tdap vaccination (Td not accepted); declinations accepted if within 1 year of start
• Negative TB test via QuantiFERON blood test within 1 year prior to start; T-Spot/PPD not accepted
• TB questionnaire required if TB positive
• Proof of immunity or vaccination for:

  • Measles (Rubeola) – 2 MMR vaccinations or positive titer
  • Mumps – 2 MMR vaccinations or positive titer
  • Rubella – 2 MMR vaccinations or positive titer
  • Varicella – 2 vaccinations or positive titer
  • Hepatitis B series, titer, or declination (declination must be within 1 year)
• Drug testing within 30 days of start using specified 9-panel bundle (no THC) with expanded opiates and oxycodone; annual update required
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/1–3/31) with timely submission of exemption requests

Background and Verification

• Comprehensive background check including:

  • National criminal search, SSN trace, sex offender, fugitive, violent offender databases
  • County searches for all counties in last 7 years
  • FACIS 3 or equivalent, SAM/EPLS, OIG, OMIG, OFAC checks
  • Education verification included
• Employment application/resume with education and work history required
• Work history verification for past 7 years through a third-party vendor; attestation letters acceptable if verification attempts exhausted
• Two professional references within 1 year of start date required; one must be a supervisor or higher (Charge RN acceptable for one reference)
• State-specific registry checks as applicable (e.g., Kentucky Vulnerable Adult Maltreatment Registry)

Facility-Specific Requirements

• Signed and dated specialty job description, renewed annually
• Completion of facility-specific employee health attestation and documentation, including:

  • TB documentation and risk assessment
  • OSHA respirator medical evaluation
  • Flu vaccine or exemption documentation during flu season
• Review and acknowledgment of facility fit test policy and safety manual
• Required submission of screenshots proving completion of health requirements in designated portals prior to start
