Location: Lafayette, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Facility Type: Short-Term Acute Care
- Shift: Monday to Friday, 5 days a week, 8-hour shifts (8:00 AM - 4:30 PM)
- Weekend Requirement: At least one weekend per month
- Holiday Requirement: Must work 2 out of 3 major holidays
- Guaranteed Hours: Options for 8- or 10-hour shifts totaling approximately 40 hours per week
- Charting System: Cerner
- Dress Code: Royal blue scrubs and black shoes
- Parking: Free
Required Qualifications
- Valid Physical Therapy Assistant license in Indiana
- Minimum 1 year of Physical Therapy Assistant experience within the last year
- Basic Life Support (BLS) certification
Compliance & Documentation Requirements
- Valid Driver’s License or Passport (clear copy required)
- Background Check: Must be completed within 90 days of start, including 7 years of residence/work history, criminal checks, sex offender registry, and education verification
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Education Verification: Official transcript or diploma required (may be pending at start)
- State License Verification: Online primary source verification within 30 days of start
- Physical Exam: Completed within 12 months of start date
- Fit Mask Test: Not required for Physical Therapists or Physical Therapy Assistants at this facility
- Immunizations and Health Screenings:
- Negative TB test or chest X-ray within 1 year
- Tetanus/Diphtheria/Tdap (strongly recommended)
- Measles, Mumps, Rubella: Positive titer or 2 vaccines required; no declinations accepted
- Varicella: Positive titer or 2 vaccines required; no declinations accepted
- Hepatitis B: Series, titer, or signed declination required
- Seasonal Flu Vaccination or approved medical/religious exemption required during flu season (9/30 - 3/31); no declinations accepted
- Drug Testing: Negative 10-panel drug screen within 12 months of start (instant tests not accepted)
- Competency Exams: Unit competency exam and medication/pharmacology competency exam required
- Color Blind Testing: Ishihara exam or equivalent with 100% minimum score required
Additional Requirements
- References: Minimum of 2 references from recent employment within last 2 years; references must be from charge, manager, supervisor, or higher
- Employment Application: Must be current and explain any employment gaps of 30 days or more over the past 7 years
- Skills Checklist: Must be completed within 12 months of start and updated annually
- Facility-specific forms and orientation packets must be completed prior to start
Notes
- Pending state licenses are not accepted; license and certifications must be in hand before starting
- Must be available for interview scheduling within 2-3 days of application
- Prior travel experience is not required for this position
