Location: Hazard, Kentucky

Job Details
- Unit: Step Down (Telemetry)
- Shift Options:
- Night shift, 3 shifts per week (12 hours each, 7:00 PM – 7:30 AM)
- Night shift, 4 shifts per week (12 hours each, 7:00 PM – 7:30 AM)
- Total Hours per Week: 36
Required Qualifications
- Valid Registered Nurse (RN) license in Kentucky
- Minimum 1 year of recent experience in a Progressive Care Unit (PCU) or Step Down Unit
- Completion of Dysrhythmia/EKG competency exam
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Health and Compliance Requirements
- Annual physical exam or physician statement within 1 year prior to start, indicating clearance to work without restrictions
- Immunizations and titers:
- Proof of 2 MMR vaccinations or positive titers for Measles, Mumps, Rubella
- Hepatitis B series, titer, or declination (declination must be within 1 year of start)
- Proof of 2 Varicella vaccinations or positive titer
- Tetanus/Diphtheria/Tdap valid within 10 years (Td not accepted; declinations acceptable within 1 year)
- Negative TB test: QuantiFERON blood test within 1 year prior to start (T-Spot/PPD not accepted)
- If positive TB history, must provide chest X-ray within last 5 years plus TB questionnaire
- Seasonal flu vaccination or approved medical/religious exemption required during flu season
- Drug screening:
- Must be completed within 30 days prior to start using specified 9-panel drug test (no THC) with expanded opiates and oxycodone
- Fit mask testing completed onsite (PAPRs not accommodated; exemptions reviewed onsite)
Background and Verification
- Background check per Appalachian Regional Bundle with expanded opiates and oxycodone panel, including:
- SSN report, national criminal search, sex offender, fugitive, violent offender databases
- County searches for all counties in last 7 years
- FACIS 3 equivalent, SAM/EPLS, OIG, OMIG, OFAC checks
- Education verification included in background or third-party verification
- NURSYS primary source verification prior to start
- State license primary source verification prior to start or upon expiration
- Work history verification covering 7 years, through third-party vendor or attestation letters if needed
- Two professional references within 1 year of start date (one must be supervisor or higher; Charge RN acceptable for one reference)
- Employment application/resume with education and work history required
Facility-Specific Requirements
- Signed and dated specialty job description, renewed annually
- Completion of orientation safety manual acknowledgement
- Compliance with employee health attestation and documentation upload requirements, including:
- TB results, TB risk assessment, OSHA respirator medical evaluation, flu vaccine or exemption documentation (screenshots required)
- Review of fit test policy and quick badge instructions
- Verification of Kentucky Vulnerable Adult Maltreatment Registry within 30 days prior to start (annual update required)
