Job Description

• Outpatient Physical Therapy position
• Work schedule options:

  • 5 days a week, 8 hours per day (5x8)
  • 4 days a week, 10 hours per day (4x10)
• Typical shift hours include:

  • 8:00 AM to 6:00 PM
  • 7:30 AM to 5:30 PM
  • 8:00 AM to 4:30 PM
  • 7:30 AM to 4:00 PM

Required Qualifications

• Valid Physical Therapist license in West Virginia
• Minimum 2 years of experience as a Physical Therapist
• Physical Therapist experience within the last year

Compliance and Documentation Requirements

• Proof of identity and eligibility to work (e.g., government-issued photo ID, I9 verification)
• Background checks including 7-year criminal history, sex offender registry, and federal exclusions (updated annually or per contract renewal)
• Drug screening (9-panel) within one year of start date; onsite drug screen may be required on day 1
• Health documents including:

  • Physical exam within the last 12 months
  • Negative TB test or chest X-ray within the last year (updated annually)
  • Immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B
  • Seasonal influenza vaccination required starting September 1st (declinations not accepted)
• Professional references:

  • 2 professional references within 2 years required for travel and allied positions
• Competency exams and skills checklists as applicable and updated annually
• State license primary source verification

Additional Notes

• Must carry photo identification during working hours
• Orientation and safety manual acknowledgment required
• Education verification may be requested
• Performance evaluations required if previously employed with the organization for over 1 year
