Case Manager (Registered Nurse) - Travel/Contract

Location: Alameda, California
Contract Length: 12 weeks
Shift: Day shift, 8:00 AM – 4:30 PM (5 days/week, 40 hours/week)

Job Summary

• Provide wound care nursing assessments including staging, measuring, photographing wounds, and communicating findings to physicians.
• Assist physicians with wound therapy and implement wound care and hyperbaric oxygen (HBO) protocols as indicated.
• Develop, document, and revise individualized plans of care in collaboration with physicians and multidisciplinary teams.
• Function as a case manager for wound care patients, coordinating home health needs, office visits, patient education, and continuity of care across settings.
• Conduct ongoing assessments, quality improvement participation, and ensure compliance with clinical pathways.
• Manage clinic operations including triage, scheduling, equipment maintenance, and infection control.
• Promote professional communication and teamwork within the clinic environment.

Required Qualifications

• Valid Registered Nurse (RN) license to practice in California.
• Basic Life Support (BLS) certification issued by the American Heart Association (AHA).
• Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association (AHA).
• Minimum 6 months of case management experience, including experience within the last year.
• Wound care experience.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Preferred Qualifications

• Certification in Wound Care (WOCN, CWS, or WCC).

Required Documents & Compliance

• Valid photo ID (Driver’s License or Passport).
• Background checks including criminal, sanction, and exclusion screenings within 90 days prior to start.
• Education verification confirming highest level of education completed.
• Physical exam within 90 days prior to start, signed by MD, DO, PA, NP, or APRN, confirming good health and ability to perform job duties.
• Immunizations and health screenings:

  • TDAP vaccine within 10 years or signed declination.
  • Negative TB test or chest X-ray updated annually.
  • Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive titers with boosters if needed.
  • Hepatitis B positive titer or signed declination.
  • Varicella vaccinations or positive titer with boosters if needed.
  • Seasonal flu vaccination or signed declination by specified deadline.
• Drug screening via oral saliva test within 30 days prior to start (rapid tests not accepted).
• Fit mask test within 90 days prior to start for patient-facing roles.
• Completion of facility-specific competencies and orientation modules on the first day of work.
• Photo for badge (passport style, no filters).
• References and employment verification covering the last 3 years of work history.
