Shift: Nights (7:00 PM - 7:30 AM), 3x12 hour shifts, weekdays, every other weekend

Job Description

• Position is for a Behavioral Health Licensed Practical Nurse (LPN) working with adult patients.
• Night shift coverage filling in for shifts not covered by regular staff.
• Holiday time-off requests limited to no more than 1 of the 3 major holidays.
• Block scheduling requests cannot be accommodated.

Required Qualifications

• Valid LPN/LVN license in West Virginia.
• Minimum 1 year of Behavioral Health experience, with experience within the last year.
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.

Required Compliance & Documentation

• Proof of I9 or attestation.
• Government-issued photo ID (must be carried during work hours).
• E-Verify proof or attestation.
• Background checks including:

  • 7-year criminal history check (all locations lived/worked in past 7 years)
  • EPLS/GSA/SAM and HHS/OIG checks (updated every 30 days)
  • Education verification or official transcript (LPN education verification required)
• WV Cares fingerprinting (results required before start, must be updated every 5 years).
• Physical exam attesting ability to meet physical demands (initial and annual).
• Immunizations and titers required or documentation of immunity for:

  • Tetanus/Diphtheria/Tdap
  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Negative TB test or chest X-ray within 1 year, updated annually.
• 9-panel drug screen within last 12 months (instant acceptable).
• Seasonal flu vaccination or approved medical/religious exemption required annually.
• Passport-style photo for badge.
• Completion of facility-specific onboarding modules prior to orientation.
• Signed facility-specific forms including media consent, flu policy acknowledgement, background disclosure, infection control questionnaire, and authorization/release forms (time sensitive).

• Scrubs provided (Navy or Royal Blue).
• Electronic Medical Record system used: Avatar.
