Job Opportunity: Registered Nurse - Case Manager
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 15 weeks
Job Description
The RN Case Manager is responsible for:
- Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience
- Completing timely Initial Assessments, updated notes, and final notes
- Completing referrals to post-acute levels of care
- Coordinating with the multidisciplinary team to ensure discharge is on track
Shift:
- 5 days a week, 8-hour shifts (Friday to Monday) with rotating weekends
- No on-call required
- Possible float to cover sick calls/PTO at other campuses if census permits
Required Qualifications
- California RN Licensure
- Minimum 2 years of RN experience with knowledge of post-acute care levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute, and Acute rehab
- EPIC experience
Certifications (as applicable)
- ACLS (AHA or ARC)
- BLS (AHA or ARC)
- Other certifications may be required depending on position specifics (verify with employer)
Background and Compliance Requirements
- 7-year background check including SSN trace, county searches, sex offender, education verification, and FACIS Level 3 screening
- Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings
- Education verification (Bachelor of Science in Nursing preferred)
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana due to CA law)
- Physical exam within 1 year of start
- Fit mask test within 1 year of start, renewed annually (with accommodations for religious exemptions or facial hair)
- Immunizations and titers required:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Negative TB test or chest x-ray within 90 days, renewed annually
- Measles, Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B series or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination during flu season (Nov-April) or signed declination
- Drug testing and immunization documentation must be current and compliant
Health and Safety Documents
- TB Screening Questionnaire within 90 days of start, renewed annually
- Respiratory Safety Clearance, renewed annually (not required for some roles)
- Color Blind Exam if applicable (in-person exam required)
Employment History and Verification
- Full 7 years of work history required with no gaps over 30 days
- Verification of employment for up to 7 years required
- Attestations for unverifiable employment accepted only for gaps less than 1 year
- References: 2 professional references required for clinical roles
Additional Notes
- Candidate must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, and other regulatory documents prior to start
- Religious exemptions for vaccines and facial hair accommodations considered on a case-by-case basis with required documentation
- No prior travel experience explicitly required for this position
