Job Opportunity: Registered Nurse (LTC) - Minnesota (Alexandria)
Position Details
- Specialty: Registered Nurse (RN)
- Location: Alexandria, Minnesota
- Employment Type: Travel/Contract
- Contract Length: 8 weeks
- Shift: 5 days a week, 8-hour day shifts (8:00 AM - 4:30 PM)
- Setting: Long-Term Care (LTC) Center
- 62 Assisted Living Beds
- 16 Memory Unit Beds
- Role: Office Nurse position involving assessments, medication management, and service plan updates/changes
- Patient Ratios: Approximately 1:20
- On-Call: Position may require 24/7 on-call availability
- Floating: Must be comfortable floating between units within the LTC setting
- Pets: Must be okay with cats and dogs on site
Experience & Requirements
- Years of Experience Required: Minimum 1.5 years as an RN
- Prior Travel Experience: Not required (First-time travelers accepted)
- Certifications Required:
- Dementia Training
- Medication Pass Experience
- BLS (Basic Life Support)
- ACLS (Advanced Cardiac Life Support) as applicable
- Licensing: Must provide proof of active RN license at time of submission
- Compliance: Must meet Aya Healthcare compliance requirements
Job Responsibilities
- Ensure health and safety of residents through assessments and reassessments
- Develop and implement service plans and plans of care
- Monitor resident services and supervise staff
- Provide ongoing staff training
- Deliver care using the Aspire Lifestyle Model focusing on residents’ social, physical, intellectual, emotional, and spiritual wellness
- Complete initial and ongoing face-to-face comprehensive assessments of residents

- Facility Type: True Long-Term Care Center
- Attire: Solid black scrubs and closed-toed shoes required
- Charting System: Eldermark
- Parking: Free parking available
- Mileage Reimbursement: None
Important Notes
- Must be able to work holidays as time off requests are only approved at submission and not after offer
- Any call-outs require more than 24 hours’ notice
- Locals are preferred but not required
Required Documentation & Compliance
- Unexpired government-issued photo ID (Driver’s License/State ID)
- Background checks including Social Security Trace, Nationwide, Sex Offender, EPLS/GSA/SAM, HHS/OIG
- Minnesota DHS NetStudy Fingerprint clearance required prior to start (candidates must complete questionnaire immediately after signing)
- State-specific mandated forms and checks (e.g., Wisconsin Caregiver Search, Iowa SING Check)
- Certifications verification (BLS, ACLS, Tech Cert, Orientation Safety Manual, CEUs)
- Health documents including:
- Physical exam within last 12 months
- Negative TB test or chest X-ray per Minnesota state guidelines
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
- 9 Panel Drug Screen within last 12 months
- Professional references (2 within last 2 years)
- Annual skills checklist and competency exams as applicable
- Facility-specific training and policy acknowledgments (e.g., dementia training, sexual relationships policy, staff disclosure policy)
This position is ideal for RNs comfortable working in a true long-term care environment with a focus on resident-centered care and comprehensive assessments. Prior travel experience is not required, making it suitable for first-time travel nurses.
