Job Details
- Start Date: ASAP
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Work: Not required
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals Accepted: Yes (within 50-75 mile radius)
- Pending License: Accepted
- Transportation: Traveler must have their own transportation
- Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
- Documentation System: EPIC Remote Client (laptop), Rover (phone app), and Oasis
- Oasis Experience: Required
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Attire: Business casual; scrubs allowed but no specific color required; no jeans
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Certifications
- BCLS (Basic Cardiac Life Support) - Must be current and from AHA or Canadian Heart & Stroke Foundation
- Other certifications may be required based on clinical track but not specified for this role
Experience Requirements
- Specialty Experience: Minimum 1 year of skilled home health experience
- Prior Travel Experience: Not required
Compliance and Documentation Requirements
- Background Check: Must be completed within 45 days of start, covering all locations lived/worked in the past 7 years; includes SSN trace, death master index, nationwide checks, MVR (required for home health), and state-specific abuse/neglect searches as applicable
- Education Verification: Required for all candidates
- Employment Application: Must include at least last 5 years of employment with month/year format; gaps or overlaps must be explained
- References: Minimum 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
- Drug Testing: 10-panel drug screen within 90 days of start (instant tests not accepted)
- Immunizations and Health Documents:
- TDAP (within 10 years) or declination
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer or declination
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer or declination
- Flu vaccine during flu season or declination
- COVID vaccine or declination
- Respiratory Safety Clearance: Required for travel candidates, must be medically cleared annually for respirator use
- Fit Mask Testing: Required for travel candidates, must be updated annually; onsite testing available if needed
Facility and System Requirements
- EPIC Training: Completion of EPIC modules required within a week of orientation
- WorkRamp Modules: Completion of required pre-start, specialty-specific, and annual regulatory training modules via WorkRamp LMS platform
- Oregon and Washington State Specific:
- Oregon Abuse/Neglect Search (if applicable)
- Washington WATCH and WSP State Patrol Searches for border facility placements
- Washington OBRA Registry Search for CNAs (not applicable for PTA)
- Float Policy: Acknowledgement required; may be asked to float to other Providence facilities within reasonable distance
