Required Credentials & Onboarding Requirements

Professional Credentials

• Current license and primary source verification within 30 days prior to assignment start
• Ascension Resume/Profile submission required
• Two professional references required prior to first assignment
• Criminal background check including Social Security Trace, Employment Verification, OIG Search, and National Sex Offender Registry (updated annually or after breaks in service)
• EPLS (Excluded Parties List System) check within 90 days of start
• Education verification relevant to clinical work within 90 days of start
• Employment verification for the past 7 years (updated annually)
• Ascension Surgical Technologist Contract Worker Onboarding Education Attestation (pre-hire modules)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) with minimum 80% score, updated yearly
• Ascension Skills Checklist (clinical specialty-specific), updated annually
• Confirmation of assignment documentation upon initial assignment and extensions

Personal Health & Immunization Requirements

• BLS certification (AHA) required before start
• Influenza vaccine required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Fit Test: Required for all travelers, preferably completed prior to start; updated annually
• 2 Step PPD or IGRA (Quantiferon/Tspot): Required with specific timing before start; TB questionnaire updated annually
• Positive PPD requires negative chest X-ray within one year and annual TB questionnaire
• Health statement or physical exam prior to first assignment, updated annually
• 12 Panel Drug Screen within 30 days prior to start, updated annually; retesting required after breaks in service

Identification

• Valid Driver’s License or State ID matching address on file; must show front and back

Additional Notes

• Flu season mask policy applies if vaccine is declined
• All required documentation must be current and compliant with Ascension Health standards
• Completion of onboarding modules and competency assessments is mandatory prior to clearance
