• 2 Step PPD or IGRA within 90 days prior to start
• Varicella vaccination or immunity proof (no declinations accepted)
• Tdap vaccination (valid for 10 years, no declinations accepted)
• 12 Panel Drug Screen within 30 days prior to start, updated annually
• Criminal Background Check including Social Security Trace, OIG Search, National Sex Offender Registry (within 30 days prior to start)
• EPLS check within 90 days prior to start
• Skills Checklist (clinical, updated annually)
• Education Verification within 90 days prior to start
• Employment Verification for past 7 years
• Confirmation of Assignment documentation
