Licensed Practical Nurse (LPN) – Long-Term Care

Location: West Burlington, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

• Provide routine daily nursing care and other services based on resident assessments, preferences, and care plans.
• Collaborate closely with the RN Manager to manage daily operations.
• Facility size: 160 beds.
• Shifts are variable, primarily evenings (2:30 PM – 11:00 PM) or nights (10:45 PM – 6:45 AM).
• Previous experience with Epic electronic health records within the last 12 months in a long-term care setting is required.
• Must hold current BLS and ACLS certifications.

Required Qualifications

• Valid LPN license for Iowa.
• BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
• Recent Epic experience in a long-term care environment (within 12 months).
• Ability to work variable shifts, including evenings and nights.

Required Onboarding Credentials and Compliance

• Professional license verification (primary source within 30 days of start).
• Completion of facility-specific skills checklist within the last 12 months.
• COVID-19 vaccination or declination accepted.
• Influenza vaccine or facility declination (seasonal, due by December 1 annually).
• MMR: 2 vaccines or positive titer; booster or declination accepted if low/equivocal.
• Hepatitis B: 2 vaccines or positive titer; booster or declination accepted if low/equivocal.
• Varicella: 2 vaccines or positive titer; booster or declination accepted if low/equivocal.
• Tdap vaccine valid within 10 years; declination accepted.
• Negative TB test within past 2 years (TSPOT, QuantiFERON IGG, or 2-step Mantoux). If prior positive, must provide chest x-ray and questionnaire.
• Fit test for approved respirators completed prior to start.
• Physician health statement within the last 12 months (updated every 4 years).
• Current driver’s license or state ID (valid throughout assignment).
• Background check within 30 days of start, including SS trace and county search for last 7 years.
• Adult and Child Abuse clearance as required by state.
• Education verification through third party or combined with diploma/transcripts.
• EPLS/GSA/SAM HHS/OIG checks within 30 days of start.
• Two professional references required, including one supervisor reference.
• Drug screen within 30 days of start (9-panel, excluding THC).
• Social Security Number inquiry and verification within 30 days.
• I-9 attestation completed on agency letterhead.
• Color vision test within 1 year.
• Contract employee data sheet.
• National Sex Offender Registry check within 30 days.
• OFAC check within 30 days.
• Pre-orientation computer-based learning (CBL) acknowledgement form.
• Epic registration acknowledgement form signed after registration completion.

Additional Information

• Resident-to-nurse ratios will be discussed during the interview.
• Facility uses specific approved respirators for fit testing (Moldex 1510, 1513; 3M 1860, 1860s, 1870+).
• All credentials and checks must be current and valid throughout the assignment.
