Schedule: 3 shifts of 12 hours each (3/12's), Friday through Sunday
Contract Length: 13 weeks

Job Description

• Prior charge nurse experience required
• Operating Room circulating nurse role
• Proficiency needed in ortho, neuro, general surgery, robotics, urology, and vascular cases

Required Credentials & Compliance

Licenses & Certifications

• Active professional nursing license verified via primary source within 30 days of start
• Nursys verification required within 30 days of start
• BLS (AHA or ARC only)
• ACLS (AHA only)
• PALS (AHA only)

Health & Immunizations

• Amsler Grid Vision Test (required before onboarding)
• COVID-19 Vaccine (optional; proof or exemption accepted)
• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures
• Influenza Vaccine: Required during flu season (10/1 - 4/30); medical/religious exemptions accepted with proper documentation
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD clearance or treatment proof required
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Vaccination within past 10 years or declination with signatures
• Annual TB Questionnaire: Required within 6 months of start; positive results require evaluation and treatment documentation
• Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+) within 1 year, updated annually
• Health Statement: Signed within 1 year confirming ability to work without restrictions
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start

Background & Drug Screening

• Criminal Background Check: Completed within 30 days prior to start, including SSN trace, county and national checks, education and employment verification (last 5 years), driving record
• Drug Screen: Negative 10-panel drug screen within 30 days prior to start (includes THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)

Competency & Skills

• FMOLHS Skills Checklist: Unit-specific, within 1 year prior to start, updated annually
• FMOLHS Competency Exam: Passing score of 80% or higher within 1 year prior to start
• FMOLHS Core Competencies: Current within 1 year prior to start, passing score 80%+, includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually
