Medical Laboratory Technician (MLT) - Travel Assignment
Location: Bloomington, Indiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Details
- Night shift position: Thursday, Friday, Saturday nights from 7:00 PM to 7:00 AM
- After training, the traveler will be the sole lab technician on site during the night shift
- Generalist role including basic blood bank, phlebotomy, and gram stain reading
Required Qualifications
- Valid Medical Laboratory Technician (MLT) license for Indiana
- Minimum of 1 year of experience as a Medical Laboratory Technician
- Medical Laboratory Technician experience within the last year
- Must meet all professional and health credential requirements prior to onboarding (see below)
Onboarding & Credential Requirements
Candidates must complete and submit the following prior to or at onboarding:
Professional Credentials
- Orientation packet completion including "Essentials" form and post-test
- Education verification (diploma or transcripts accepted if third-party verification unavailable)
- Driver’s license, state ID, or passport
- National criminal background check within one year of start date
- Employment eligibility verification (Form I-9 or agency attestation)
- Resume reflecting accurate and recent work history (required at offer acceptance)
- Two professional references, including at least one from a manager or direct supervisor
- Completion of core competencies exam and specialty exam (minimum 80% score)
- Skills checklist
- Agency attestation form
- Search of excluded individuals database (SAM/EPLS)
- Workplace violence prevention training (DCI/BVP)
Personal Health History & Immunizations
- COVID-19 vaccination series plus booster or completed declination
- MMR: proof of two vaccines or positive immunity titer
- Annual respirator fit test (must be completed prior to start if onsite testing not available)
- Hepatitis B vaccination (declinations not accepted at some locations)
- Influenza vaccine or declination (required during flu season)
- Tuberculosis test within one year of start date
- Varicella vaccination or immunity titer
- Tdap vaccination (declinations not accepted at some locations)
- Physical/health statement confirming clearance to work (completed annually or within 60 days of start)
- Drug screening (minimum 10-panel test including THC) within 60 days of start date
Additional Testing
- Annual color-blind test
- Positive PPD with negative chest X-ray (optional)
- Badge photo (optional, facility-specific)
- Name verification affidavit if discrepancies exist between credentials
