Job Description

• Growing Per Diem Pool
• Start Date: January 17, 2026
• Candidates must be available on weekends
• Preference given to candidates available for a full schedule through the end of January and the first week of February
• Minimum compliance requirements must be met prior to starting
• Full compliance profile must be completed within two weeks after the start date

Required Credentials & Compliance Items

Licenses & Certifications

• Professional license verification (Primary Source Verification within 30 days of start)
• BLS (American Heart Association or Red Cross)
• ACLS (American Heart Association or Red Cross)
• PALS/NRP as required
• DCI/BVP (Workplace Violence Prevention training)
• Prime Core Competencies Exam (must score 80% or higher; attestation accepted)
• Prime Skills Checklist
• Prime Criminal Background Check (National, within one year of start date)
• Prime SAM/EPLS exclusion search
• Prime Form I-9 (Employment eligibility verification)
• Signed Prime Job Description

Health & Immunizations

• COVID-19 Vaccine: 2 doses plus booster or completed declination
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Varicella: Vaccination or titer showing immunity
• Hepatitis B: Vaccination or declination accepted
• Influenza Vaccine: Required for Nov 1, 2025 – Mar 31, 2026 (declinations accepted except at certain facilities)
• Tdap: Vaccination or declination accepted
• TB Test: Within one year of start date
• Positive PPD with negative chest X-ray (if applicable, every 4 years)
• Physical/Health Statement: Annually, must state clear to work

Drug Screening & Background

• Drug Screen: Minimum 10-panel test WITH THC included, within 60 days of start

  • For California locations, requires both a 10-panel urine drug screen excluding THC and a psychoactive THC saliva test
• Background Check: National Criminal Background Check within one year of start date

Additional Requirements

• Driver’s License, State ID, or Passport
• Prime CA Sexual Harassment Prevention Training (valid for 2 years)
• Prime Agency Attestation (completed after all credentials uploaded)
• Prime Resume (up-to-date at time of offer acceptance)
• Two professional references required (one must be from a manager or direct supervisor)
• Prime Color-blind test (annual, Relias accepted)

Optional

• Annual Respirator Fit Test (if applicable, must be completed prior to start or coordinated onsite where allowed)
• Name Verification Affidavit (if name discrepancies exist)
