Licenses & Certifications

• Nursys License Verification (Primary source verification required within 30 days of start)
• BLS (American Heart Association or Red Cross)

• Prime Orientation Packet (Includes Essentials Form and Post Test)
• Prime Core Competencies Exam (Must score 80% or higher)

Personal Health History & Immunizations

• COVID-19 Vaccine (2 vaccines plus booster required or declination completed)
• MMR (Proof of 2 vaccines at least 28 days apart or positive titer)
• Hepatitis B (Declinations not accepted)
• Influenza Vaccine (Required November 1 - March 31; declinations not accepted)
• TB Test (Quantiferon or T-Spot within past year with negative result OR two negative PPD skin tests within past year)
• Varicella (Proof of 2 vaccines at least 28 days apart or positive titer)
• Tdap (Declinations not accepted)
• Physical/Health Statement (Must be completed within 60 days of start, stating clear to work)
• Prime Drug Screening (Minimum 10-panel test including THC within 60 days of start; CA locations require additional saliva THC test)
• Positive PPD - Negative Chest X-Ray (If applicable, negative chest x-ray within 3 months of positive test required)
• Fit Test (Annual respirator fit test; must be completed prior to start if not available onsite)
• Prime Color-blind Test (Annual, Relias accepted)

• Prime Badge Photo (Clear headshot, JPEG, may be requested for onboarding)
• Name Verification Affidavit (If name discrepancies exist between credentials)
