Physical Therapist - Travel Position

Location: Wisconsin Rapids, Wisconsin
Contract Length: 13 weeks

Job Overview

• Provide home health physical therapy services to adult and geriatric patients with orthopedic, neurological, and functional mobility diagnoses.
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice.
• Document care to meet regulatory and reimbursement requirements using NetSmart EMR.
• Productivity expectation: average of 4.5 patient visits per day.
• Work schedule includes 8 variable days Monday through Friday, with occasional travel to central and northern regions.
• Rotate one Saturday every 6-8 weeks, with a weekday off during those weeks. No holiday shifts.
• Guaranteed 38 hours per week, flexible up to 40 hours.

Required Qualifications

• Valid Physical Therapist license for the state of Wisconsin.
• Bachelor's Degree in Physical Therapy (Master’s degree knowledge expected).
• Home Health experience working with adult and geriatric populations.
• Certification: AHA or ARC Basic Life Support (BLS).
• Valid driver’s license with proof of car insurance.
• National Provider Identifier (NPI) number required or must begin application process prior to start.

Additional Information

• Must live outside a 50-mile radius of the primary location.
• Use of laptop for point-of-service documentation and cell phone for GPS and communication required.
• On-call status possible during low census periods; call-back minimum is 2 hours with pay.
• Up to 36 hours of unbillable low census hours allowed per contract.
• Flu season policy: mandatory mask-wearing for travelers who decline flu and COVID vaccinations; non-compliance may result in corrective action.
• Dress code: business casual or any scrub colors.

Shift Details

• Typical shift: 8:00 AM – 4:30 PM (8 hours per day).
• Occasional travel required within the region.
