Location: Detroit, Michigan
Contract Length: 17 weeks
Job Summary
- Provide direct patient care in the home under general supervision of the nursing supervisor.
- Assess need for allied health care services (physical therapy, occupational therapy, nutrition, speech pathology, social work) to ensure integrated home health care delivery.
- Support patient load in Wayne, Oakland, and Macomb counties with approximately 6 patient visits per day.
- Drive an average of 40-60 miles per day.
- Orientation lasts 2-4 weeks, conducted both onsite and in the field with a preceptor.
Work Schedule
- Shift: 8:00 AM to 4:30 PM
- One Saturday per month
- 40 hours per week
Required Qualifications
- Valid RN license in Michigan
- Minimum of 1 year of Home Health experience within the last year
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing or Associate Degree in Nursing
- Experience in a Home Health setting
- Valid Michigan driver’s license with a satisfactory driving record
- Reliable transportation and telephone
- Ability to lift, carry, transport, and move clients as needed
- Strong interpersonal, communication, and customer service skills to interact effectively with diverse populations
Preferred Qualifications
- Acute care experience.
