RN - Home Health

Location: Detroit, Michigan
Contract Length: 17 weeks

Job Summary

• Provide direct patient care in the home under general supervision of the nursing supervisor.
• Assess need for allied health care services (physical therapy, occupational therapy, nutrition, speech pathology, social work) to ensure integrated home health care delivery.
• Support patient load in Wayne, Oakland, and Macomb counties with approximately 6 patient visits per day.
• Drive an average of 40-60 miles per day.
• Orientation lasts 2-4 weeks, conducted both onsite and in the field with a preceptor.

Work Schedule

• Shift: 8:00 AM to 4:30 PM
• One Saturday per month
• 40 hours per week

Required Qualifications

• Valid RN license in Michigan
• Minimum of 1 year of Home Health experience within the last year
• BLS certification issued by the American Heart Association
• Associate of Science in Nursing or Associate Degree in Nursing
• Experience in a Home Health setting
• Valid Michigan driver’s license with a satisfactory driving record
• Reliable transportation and telephone
• Ability to lift, carry, transport, and move clients as needed
• Strong interpersonal, communication, and customer service skills to interact effectively with diverse populations

Preferred Qualifications

• Acute care experience.
