Job Description

• Work in a long-term care skilled nursing facility with a 160-bed capacity.
• Provide skilled nursing care including medication administration, IV therapy, feeding tubes, wound vac and wound management, tracheostomy care, ostomy care, therapy, and restorative care.
• Patient age range: mid-30s to mid-90s.
• Common diagnoses include Huntington’s disease, dementia, diabetes, Alzheimer’s, coronary artery disease, vascular disease, rehabilitation post-stroke and TIA, neuromuscular diseases.
• Dialysis is performed at an outpatient facility.
• Utilize computerized medical record documentation systems and medication administration systems.
• Equipment used includes vital signs towers, INR monitors, glucometers, lifts, bladder scanners, IV pumps, CADD pumps, wound vacs, sequential compression devices (SCD), and assistive devices.
• Nursing shifts are 12 hours with certified nurse aide support.
• Nurse-to-patient ratio: approximately 20 patients if administering all medications; up to 45 if medication aides are present.
• May be responsible for medication administration for up to 20 residents.

Required Skills and Experience

• Minimum 2+ years of nursing experience preferred.
• Experience in MedSurg and Geriatrics preferred.
• IV certification preferred.

Required Licenses and Certifications

• Valid Wyoming or compact LPN license.
• Basic Life Support (BLS) certification required.

Schedule

• All shifts and weekends required based on facility needs.
• Support staff available including certified nurse aides.
• Orientation program: 2 days.

Dress Code and Appearance

• Navy blue scrubs required.
• No facial piercings allowed.
• Full sleeve or objectionable tattoos must be covered.

This position is suitable for LPNs with experience in long-term care and skilled nursing environments who are comfortable managing complex patient needs and working flexible shifts including weekends.
