Registered Nurse (RN) - Long Term Care / Skilled Nursing Facility

Location: Winnemucca, Nevada
Contract Length: 13 weeks

Facility Overview

• Nursing home with skilled care attached to a critical-access hospital
• 34 beds, currently 22 residents, all semi-private rooms
• EMR system: Cerner
• Common diagnoses: COPD, diabetes, cognitive deficits, multiple comorbidities

Staffing Ratios

• Day shift RN/LPN ratio: 1:11
• Night shift RN/LPN ratio: 1:22 or 1:30
• CNA ratio days: 1:6
• CNA ratio nights: 1:6 or 1:8

Job Duties

• Medication administration, simple IVs, and tube care (no vents or trachs)
• Prefer candidates with leadership experience such as charge nurse duties
• Wound care managed by PT; nurses assist as needed
• Interaction with residents with cognitive deficits; ability to redirect without confrontation is important
• No titratable drips or telemetry monitoring required

Schedule & Shifts

• 12-hour shifts, typically 7:00 AM to 7:30 PM
• Day shift staffing: 2 nurses and 4 CNAs
• Night shift staffing: 1 nurse and 3-4 CNAs
• Modified self-scheduling; flexibility required to fill staffing gaps
• Weekend work typically every other weekend
• Possible holiday rotation participation
• No call or standby requirements
• Floating to memory care unit as needed

Orientation

• One shift of orientation with a preceptor

Team & Support

• Interdisciplinary team includes PT, OT, SLP, DON, Resident Care Coordinator, MDS Coordinator, Resident Advocate, Dietician, NP, and Medical Director as needed
• Physician oversight available after hours; NP available Monday-Friday and on-call after hours
• Respiratory therapy administered by nurses; ER available 24/7
• Pharmacy on-site with on-call coverage at night

Required Certifications & Qualifications

• Valid RN license in Nevada
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 1 year of rehabilitation experience, preferably within the last year
• Experience in nursing home/skilled nursing care
• BLS certification
