Job Opportunity: MDS Coordinator (Minimum Data Set Coordinator)

Location: Lakota, North Dakota

Job Summary

Provide care to assigned residents in a safe, caring, and efficient manner. Responsible for completing the nursing portion of the Minimum Data Set (MDS) assessments, ensuring accuracy and compliance with standards and individualized resident care plans.


Essential Duties and Responsibilities

  • Complete assigned portions of the MDS for Nursing and sign off on forms.
  • Maintain residents’ self-esteem, privacy, and confidentiality.
  • Assist in maintaining safe, pleasant, and orderly resident living areas.
  • Review and validate the entire MDS process for accuracy.
  • Communicate resident conditions and updates with physicians and care team members.
  • Educate and support co-workers, residents, and families regarding the RAI/MDS process.
  • Use Electronic Medical Records (EMR) accurately.
  • Comply with all relevant laws, regulations, and company policies.

Required Qualifications

  • Graduate of an accredited school of nursing.
  • Current RN license valid in North Dakota.
  • Certification as a Resident Assessment Coordinator (RAC-CT) through AANAC within 6 months of hire.
  • Knowledge of the RAI/MDS process and basic nursing skills.
  • Ability to communicate effectively in English, both orally and in writing.

Physical and Work Environment Requirements

  • Ability to lift up to 25 lbs.
  • Ability to maintain stationary positions and move as needed to perform job tasks.
  • Work shifts of 8 hours; may cover additional shifts.
  • No weekends or holidays required.
  • Moderate noise level in the work environment.
  • Frequency of travel may vary based on business needs.
