Required Qualifications

• Valid Licensed Clinical Social Worker license in California
• Licensed Clinical Social Worker with 2 years of experience
• Licensed Clinical Social Worker experience within the last year
• Basic Life Support (BLS) certification
• California 5150 certification

Interview / Submission Requirements

• 2 supervisory references required at submittal, including 1 within the last 12 months and 1 within the last 3 years worked
• Copy of driver’s license required at submittal to validate legal name; name must match a government-issued ID and must not include a middle initial or middle name

Employment Restrictions

• Candidates who have been directly employed by any facility associated with CommonSpirit, including CHI and Dignity, within the past year are not eligible
• Candidates previously employed PRN through outside firms are eligible

Dress Code

• Business casual attire
• White lab coat provided by the unit for care coordination
