Minimum Qualifications
<ul><li>Associate degree in Physical Therapy from an accredited educational program.</li><li>Active State License is Required to Start the Assignment</li><li>BLS Certification May Be Required from AHA or ARC</li><li>PTA licensure in IL</li></ul>
 
Assignment Details
<ul><li>Schedule: M-F 8:00 - 4:30</li><li>On-call or weekend requirement: No</li><li>Coverage area: Agency service area</li><li>Mileage reimbursed between visits, from first patient to last patient</li></ul>
