Job Summary

Required Experience

• Vascular experience including EVAR, TVAR, bypass, AAA, angiograms, carotids
• Thoracic experience with robotics
• Cardiovascular experience: Minimum 2 years required

Personal Health History & Immunizations

• Amsler Grid Vision Test (required before onboarding)
• COVID-19 Vaccine (optional; proof or exemption accepted)
• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+), completed within 1 year prior to start, updated annually
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures
• Influenza Vaccine: Required during flu season (10/1 - 4/30); medical or religious exemptions accepted with proper documentation and approvals
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD note or proof of treatment required
• Health Statement: Signed and dated within 1 year confirming ability to work without restrictions
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required signatures
• Annual TB Questionnaire: Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• FMOLHS Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start

Drug Screening & Testing

• FMOLHS Drug Screen: Negative 10-panel drug screen within 30 days prior to start (includes marijuana/THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)
• FMOLHS Skills Checklist: Unit-specific, within 1 year prior to start, updated annually
• FMOLHS Competency Exam: Passing score of 80% or higher within 1 year prior to start
• FMOLHS Core Competencies: Current within 1 year prior to start, passing score of 80% or higher; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually
