Shift: 36 or 48 hours per week

Required Credentials & Onboarding Requirements

Licenses & Certifications

• Nursys Verification: Must be completed within 30 days prior to start. If any disciplinary actions, complete FACIS Questionnaire.
• Current Nursing License: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
• BLS (AHA): Required prior to onboarding.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
• Driver’s License/State ID: Must be current, match address on file, and show front/back.

Testing & Exams (Must be completed prior to start)

• Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years.
• Ascension EKG Exam: Minimum 80% score, max 2 attempts, latest edition required.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max 2 attempts, completed within a year and updated yearly.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
• Ascension Skills Checklist: Clinical skills checklist, completed within one year of start and updated annually.

Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer; no declinations accepted. If negative titer, two doses four weeks apart required.
• Varicella: Proof of two vaccines, serological immunity, or documented history; no declinations accepted. If negative, two doses four weeks apart required.
• Tdap: Valid within 10 years; no declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted but mask required.
• 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start; or negative IGRA within 90 days. TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year.
• Fit Test: Required for all travelers; preferred before start, can be done onsite (except PAPR). Annual update required.
• Health Statement: Statement of good health or physical exam prior to first assignment; updated annually.
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified substances. Retesting required if break in service > 90 days.

Professional Documentation

• Resume/Profile: Required for submission.
• Two Professional References: Required prior to first assignment.
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days of assignment and updated annually or after >90-day break.
• EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days.
• Education Verification: Relevant to clinical work, completed within 90 days of start; rerun if break in service > 90 days. Official transcripts required if verification fails.
• Employment Verification: Past 7 years, third-party verification required; updated annually or after >90-day break.
• Confirmation of Assignment: Provided upon initial assignment and updates with extensions; must be on agency letterhead with accurate dates.

Onboarding & Training

• Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire training with attestation upload.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
• Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations or filters.

Optional

• Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
• Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended for those with positive TB test history.
• Hiring Manager Evaluation: Post-assignment evaluation.
• Ascension Name Change Document: Legal documentation of name change.
