Job Opportunity: LPN Home Health

Location

• Long-Term Assignment (approximately 13 weeks)

Schedule

• Monday to Friday, 8:00 AM – 4:30 PM
• One weekend shift required per month (rotating 1 out of 4 weekends)

Job Description

• Provide skilled nursing care in patients' homes, performing approximately 6 patient visits per day.
• Work collaboratively within interdisciplinary teams including RNs, Physical Therapists, Occupational Therapists, and Home Health Aides.
• Deliver care as delegated by RN Case Manager or Clinical Team Manager.
• Must have prior home care experience.
• Practice in accordance with the State Nurse Practice Act, professional standards, and applicable laws.

Required Qualifications

• Licensed Practical Nurse (LPN) license valid in Massachusetts (no conditional licenses accepted).
• Home care experience required.
• Basic Life Support (BLS) certification (AHA preferred; Red Cross accepted).
• Current N95 respirator fit test (within 1 year) and annual updates.
• Valid Driver’s License or State ID.
• Documentation of immunizations or immunity for Measles, Mumps, and Rubella (MMR).
• Tuberculosis (TB) screening within 3 months of start date (skin test or blood test) and updated if there is a break in service longer than 6 months.
• Physician health statement dated within 1 year of start.
• Completion of respirator medical clearance attestation.
• Background checks including:

  • Criminal background check within 45 days prior to start.
  • Massachusetts CORI check within 30 days prior to start.
  • Motor Vehicle Record (MVR) check within 45 days prior to start.
  • Social Security Number trace covering last 7 years.
  • National sex offender registry check within 30 days prior to start.
  • Exclusion checks against federal and state sanction lists within 30 days prior to start and annually.
• Submission requirements:

  • Resume with 7 years of work experience including city and state.
  • Two professional references within 4 years (minimum one supervisor-level).
  • Skills checklist completed within 1 year.
  • Candidate submission coversheet.
• Additional onboarding requirements:

  • Color photo (no filters, plain background, no eyeglasses).
  • Confirmation of assignment form.
  • Education verification (highest level achieved).
• Must comply with all organizational policies and procedures.

Additional Notes

• No religious or medical exemptions allowed for N95 fit testing.
• Fit testing cannot be performed onsite; must be completed prior to start.
• TB positive candidates require a negative chest x-ray dated after positive test.
• Prior travel experience is not specified as required for this position.
