Job Opportunity: Medical Laboratory Technician (MLT)

Location: Canal Winchester, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• 8 or 12 hour day shifts
• Every other weekend
• Rotating holidays

Required Certifications and Licenses

• BLS (American Heart Association certification) – recommended for direct patient care personnel
• Current state license/certification verification (must be dated within 30 days of start)

Required Onboarding Credentials and Documentation

• Fit Test (specific to certain states)
• Signed Trinity Code of Conduct Form
• Completed Trinity I9 form
• Confidentiality and System Security Agreement
• Integrity and Audit Services Form
• Specialty Specific Competency Testing (must be completed within the last year)
• Color Vision test
• 2 Step PPD OR IGRA (Quantiferon/Tspot) test
• Skills Checklist (provided by agency for clinical contractors)
• Proof of MMR vaccination or immunity
• Hepatitis B vaccination series (must have started series prior to start)
• Proof of Varicella vaccination or immunity
• Current Tdap vaccine or declination form (declination accepted if no pediatric interaction)
• Physical/Health Statement (exam within the last year confirming fitness to work)
• Conflict of Interest Disclosure Form
• Criminal Background Check (federal, nationwide, county for last 7 years, including SSN verification; must be dated within 30 days of start)
• Education Verification (highest academic degree or equivalent)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• System Access Form (must be completed and uploaded one week before start)
• Joint Commission CORE Exam (within the last year)
• COVID-19 Vaccine proof, booster by Sept 2024, or approved declination/exemption (declination not accepted in CA or IL)
• Influenza Vaccine proof or approved declination/exemption (declination not accepted in CA or IL; flu season 10/1/2025 - 4/30/2026)
• 10 Panel Drug Screen (completed within 30 days of start; includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates)
