Required Qualifications

• Valid RN license in Pennsylvania
• Long Term Care experience within the last year
• 1 year of Long Term Care experience
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS issued by American Red Cross or American Heart Association
• Government-issued photo ID
• Background check within 1 year of start covering all locations lived/worked in the past 7 years
• EPLS/GSA/SAM within 30 days of start
• HHS/OIG within 30 days of start
• Pennsylvania Department of Human Services fingerprint results
• NURSYS verification for RNs
• State license primary source verification
• Physical exam
• Negative TB test or chest X-ray within 1 year of start
• 9-panel drug screen within the last 12 months
• Proof of 2 clinical references
• Complete application/resume
• Skills checklist for applicable unit
• Medication test/pharmacology competency exam
• Competency exam

Health Documentation

• Rubeola: titer or 2 vaccines
• Mumps: titer or 2 vaccines
• Rubella: titer or 2 vaccines
• Hepatitis B: titer, vaccine series, or declination
• Varicella: titer or 2 vaccines
• TB questionnaire for past positive TB history or in lieu of annual TB test renewal
