Overview

  • Travel/contract role in Burlingame, California
  • Day shift: 5x8 hours, 8:00 AM to 5:00 PM
  • Guaranteed 40 hours per week
  • Average census: 5–6 patients per day
  • Home health caseload includes gait, strength, and range of motion
  • Territory covers San Francisco, primarily downtown

Required Qualifications

  • Valid Physical Therapist license in California
  • Physical Therapist (1 year of experience)
  • Physical Therapist experience within the last year
  • BLS - Basic Life Support issued by American Heart Association or American Red Cross
  • Experience in a Home Health setting
  • OASIS experience

Additional Requirements

  • References from previous home health or hospice agencies
  • Comfortable working in a heavily populated urban area
  • Local candidates accepted within a 50-mile radius

Work Conditions

  • On-call not required
  • Mileage reimbursed at IRS standard
  • Dress code: scrubs, any color

Documentation / Compliance

  • Government-issued photo ID
  • Emergency contact form before start
  • Background check within 1 year of start
  • EPLS/GSA/SAM screening annually
  • HHS/OIG screening annually
  • Physical exam within 1 year of start
  • TB screening within 1 year of start
  • Measles, mumps, rubella, hepatitis B, varicella, and flu documentation required per source policy
  • Drug screen within 1 year of start
  • Professional references required within 3 years of start
  • Skills checklist required annually
