Overview
- Travel/contract role in Burlingame, California
- Day shift: 5x8 hours, 8:00 AM to 5:00 PM
- Guaranteed 40 hours per week
- Average census: 5–6 patients per day
- Home health caseload includes gait, strength, and range of motion
- Territory covers San Francisco, primarily downtown
Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist (1 year of experience)
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- Experience in a Home Health setting
- OASIS experience
Additional Requirements
- References from previous home health or hospice agencies
- Comfortable working in a heavily populated urban area
- Local candidates accepted within a 50-mile radius
Work Conditions
- On-call not required
- Mileage reimbursed at IRS standard
- Dress code: scrubs, any color
Documentation / Compliance
- Government-issued photo ID
- Emergency contact form before start
- Background check within 1 year of start
- EPLS/GSA/SAM screening annually
- HHS/OIG screening annually
- Physical exam within 1 year of start
- TB screening within 1 year of start
- Measles, mumps, rubella, hepatitis B, varicella, and flu documentation required per source policy
- Drug screen within 1 year of start
- Professional references required within 3 years of start
