Physical Therapist - Acute Care Hospital (Travel/Contract)
Location: Fort Worth, Texas
Shift: Day, Monday-Friday, 8:00 AM – 4:30 PM (40 hours/week)
Contract Length: 13 weeks
Job Description
- Inpatient physical therapy at main campus acute care hospital.
- Standard daytime hours, Monday through Friday.
Required Qualifications
- Valid Physical Therapist license in Texas
Preferred Qualifications
- Minimum 1 year of experience as a Physical Therapist.
Onboarding & Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID).
- Background check including SS trace, county searches for last 7 years, nationwide and sex offender registry.
- Education verification of initial and most recent related degree.
- Physical exam within 1 year of start date.
- Negative TB test (TSpot or QGold) within 1 year; PPD not accepted. If positive TB, negative chest X-ray within 1 year required.
- Required immunizations or proof of immunity:
- Rubeola (Measles) – positive titer or 2 vaccines (boosters if low titer).
- Mumps – positive titer or 2 vaccines (boosters if low titer).
- Rubella – positive titer or 2 vaccines (boosters if low titer).
- Varicella – titer, 2 vaccines, or history of disease (boosters if low titer).
- Hepatitis B – titer, declination, or vaccine series (3 doses).
- Tetanus/Diphtheria/Tdap – TDAP required, TD not accepted; exemptions require facility-specific form and approval.
- Seasonal flu vaccination or approved medical/religious exemption required year-round.
- Negative 10-panel drug screen within 30 days of start (instant tests not accepted).
- Two professional references verified within one year of start.
- Annual skills checklist updated and valid at start date.
Additional Notes
- All onboarding documents and forms must be completed electronically prior to documentation due dates.
- Occupational health appointment required before or on first day for final clearance.
- Facility-specific orientation includes pass/fail testing (e.g., medication calculation).
- COVID-19 vaccination is highly encouraged but not mandatory; no exemption forms required.
