OT Home Health - Travel/Contract Position

Location: San Ramon, California
Contract Length: 13 weeks

Job Details

• Setting: Home Health
• Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week
• Weekly Hours: 36 guaranteed hours (traveler expected to see 5-6 patients daily; guaranteed hours not applied during first 2 weeks, holidays, or time off weeks)
• Patient Ratio: 1:1, may visit up to 6 patients per day
• Weekend Work: Optional to meet productivity requirements
• On-Call: Not required
• Travel Radius: Occasional travel up to 50+ miles; system groups clients to minimize drive time
• Transportation: Must have own reliable transportation
• Scrub Color: Any color scrubs allowed
• Parking: Free parking available
• Mileage Reimbursement: IRS standard

Required Qualifications

• Valid Occupational Therapist license in California
• Minimum 1 year of Occupational Therapist experience within the last year
• Experience in a Home Health setting
• Current Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form
• I9 Attestation
• Background checks including 7-year work/living history, sex offender registry, and federal exclusions (updated annually)
• Current physical exam within 1 year, cleared to work without restrictions
• Immunizations and titers as applicable:

  • Tdap (within 10 years or annual declination)
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
• Negative TB test or chest X-ray within 1 year
• 10-panel drug screen within 1 year
• Flu vaccine or declination required year-round
• COVID-19 vaccine documentation or declination form required
• Auto insurance and registration (must be current throughout assignment)
• Professional references:

  • 2 for travel, allied, or per diem acute care
  • 1 for per diem non-acute care
• Work history verification of 1 year related employment
• Annual skills checklist and competency exams as applicable

Additional Notes

• Timely communication and documentation are required; documentation tools will be provided and must be returned
• Locals are accepted for this travel assignment

This position is ideal for experienced Home Health Occupational Therapists who have prior travel contract experience and are licensed in California.
