Physical Therapist - Home Health

Location: Eureka, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Specialty: Home Health Physical Therapy
• Minimum 1 year recent experience required in specialty
• Average patients per day: 5-7 (to be confirmed during interview)
• Productivity expectations: 6 visits daily and minimum 30 points weekly
• Types of cases: Geriatric
• Guaranteed hours: 40 per week
• On-call required every 3rd to 4th weekend
• Territory: Up to 60-mile radius from office/hub
• First-time travelers accepted
• Locals accepted at travel rate
• Dress code: Scrubs (any color), no jeans
• Documentation and communication must be timely and accurate
• Device provided: iPad (cell phone use not allowed)
• Mileage reimbursed at IRS standard rate
• Flexibility with territory expected

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of recent experience in Physical Therapy Home Health
• Basic Life Support (BLS) certification required
• Experience with Homecare Homebase

Compliance & Background Requirements

• Background checks within 90 days of start, including:

  • SSN Trace, County searches (last 7 years), FACIS L1M, Nationwide, Sex Offender, Motor Vehicle Records, Education Verification, Work History Verification
• EPLS/GSA/SAM, HHS/OIG checks required within 90 days of start
• Fingerprinting as required by state regulations
• Education verification of highest degree related to profession
• State-specific criminal history and employability status checks as applicable
• Physical exam within last 12 months (not required annually)
• Fit mask test (N95 accepted)
• Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
• Required immunizations or proof of immunity:

  • Measles (Rubeola), Mumps, Rubella
  • Hepatitis B
  • Varicella
• Drug screening: 9-panel drug test within last 12 months
• Seasonal flu vaccination or facility-specific declination form required
• COVID vaccination or facility-specific declination required

Additional Requirements

• Proof of current auto insurance required due to driving responsibilities
• Must complete orientation safety manual acknowledgment annually
• References: 2 professional references within 2 years of start
• Employment application with full 7 years of work history, no gaps
• Annual skills checklist and competency exams as applicable

Notes

• Candidates must have a flexible approach to territory coverage
• Timely and accurate documentation is critical
• Scrubs required; no jeans allowed
• iPad provided for documentation; personal cell phones not permitted on job

This position is suitable for Physical Therapists with at least 1 year of recent home health experience who are comfortable with travel assignments and on-call weekend rotations.
