Job Description
- Provide physical therapy services primarily in patient homes and some care facilities.
- Average patient load: 5-6 patients per day.
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts).
- Must be willing to travel up to 75 miles from the home clinic.
- Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
- Use of EMR systems including HomeCare HomeBase, Oasis, and Outlook.
- No on-call required for this position.
- Local candidates accepted.
Required Qualifications
- Minimum 1 full year of recent Adult Home Health experience (first-time home health therapists not accepted).
- Current Physical Therapist license in hand (pending licenses not accepted unless only a few days to obtain).
- CPR and BLS certification required.
- Reliable transportation and current driver's license required.
- Agency-required liability insurance required.
Additional Requirements & Compliance
- Must complete standard background checks including 7-year history, sex offender, and state-specific searches.
- Drug screening (10-panel) required.
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months.
- Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination documentation required.
- Completion of various compliance and confidentiality documents prior to start.
- Must review and sign agency-required policies and training materials annually or at assignment start.
- Two professional references required.
- Verification of work history for the last 7 years or at least two most recent employers.
Travel & Schedule Notes
- Patient visits primarily in home health settings with some facility visits.
- Travel expected daily between patient homes (40-50 miles per day).
- Schedule and weekend requirements may vary; discuss during interview.
