Location: Walla Walla, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Specialty: Home Health Physical Therapy
- Minimum 1 year recent experience required
- OASIS and HCHB EMR experience required
- Average caseload: 5-7 patients per day (to be confirmed during interview)
- Guaranteed 30 visits weekly with a 40-hour weekly guarantee
- All charting must be completed by 9 AM the following morning
- Patient population primarily geriatric
- First-time travel therapists not accepted
- Territory covers up to a 60-mile radius from the office/hub
- Locals accepted at travel pay rate
- Timely documentation and communication required
- Dress code: Scrubs (any color), no jeans
- iPad provided for work; cell phone use not allowed during visits
- Mileage reimbursed at IRS standard rate
Required Qualifications
- Valid Physical Therapist license in Washington
- Minimum 1 year of recent Physical Therapist experience within the last year
- Experience with Homecare Homebase (HCHB) EMR system
Compliance and Background Requirements
- Background checks within 90 days of start, including:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1
- Nationwide Sex Offender
- Motor Vehicle Records/Driver’s Record
- Education verification
- Work history verification (7 years)
- EPLS/GSA/SAM checks within 90 days of start
- HHS/OIG checks within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Employment application with full 7 years of work history, no gaps
- Two professional references within 2 years of start
Certifications and Competencies
- BLS (Basic Life Support)
- Orientation Safety Manual Acknowledgement (updated annually)
- State license primary source verification within 90 days of start and upon expiration
- Medication test/pharmacology competency exam as applicable
- Competency exam as applicable to specialty
Health and Immunization Requirements
- Physical exam within last 12 months (not required annually)
- Fit test for N95 mask (no preferred model)
- Negative TB test or chest X-ray within 1 year of start (chest X-ray within 5 years if positive PPD)
- Annual TB questionnaire required
- Immunizations or positive titers for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Flu vaccine or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
- 9-panel drug screen within last 12 months (instant acceptable)
Additional Requirements
- Proof of current auto insurance required due to driving responsibilities
- Must complete facility-specific acceptable use policy prior to start
- Must complete facility-specific flu and COVID declination statement if declining vaccines

- Prior travel experience is required; first-time travelers are not accepted for this position.
- Timely and accurate documentation is critical.
