Provide skilled home health therapy visits (minimum 6-8 visits per day).
Patient populations include Cardiopulmonary, Geriatrics, Neurology, Orthopedics, and Oncology specialties.
Responsibilities include assessment, documentation, use of equipment/devices (crutches, gait belts, walkers, supplemental oxygen, etc.), home exercise programs, medication management, patient and family education, and safety/injury prevention.
Must have home health experience; OASIS training preferred.
Required to work every 3rd weekend.
Visits and hours are self-scheduled based on office needs.
Use of laptop, MiFi for internet, and portable printers provided.
Documentation via BayCare Forms, Scheduling, and Ace Timesheet systems.

Minimum 2 years of relevant experience.
Must hold a valid state license.
BLS certification required.
Must have a valid driver’s license and reliable car for home visits.
COVID-19 vaccination required or approved exemption on file.
Flu shot required or declination on file; mask wearing mandatory at all times while working.

BLS (Basic Life Support) - AHA or RQI accepted.
ACLS (Advanced Cardiac Life Support) - AHA or RQI accepted.
PALS (Pediatric Advanced Life Support) - AHA or RQI accepted.
NRP (Neonatal Resuscitation Program) - as required.
EFM (Electronic Fetal Monitoring) - as required.
National tech certifications as applicable (e.g., RRT, ASCP, RCIS).
Stroke Education CEUs (8 total) including NIH Stroke Certification and mRS (Modified Rankin Scale) for relevant units.
CPI (Crisis Prevention Institute) for Behavioral Health or Psych ER roles.
Competency exams with minimum passing scores (e.g., medication/pharmacology, dysrhythmia/EKG for certain units).
State license primary source verification.
NURSYS verification for RNs and LVNs.

Background check within 1 year including 7 years of residence/work/school history.
EPLS/GSA/SAM and HHS/OIG checks required.
Foreign equivalency evaluation required for international education.
Education verification required for all healthcare professionals.
Florida AHCA Level II fingerprinting clearance required prior to start.
Drug testing: Standard 9-panel within 30 days of start (instant tests not accepted).
Physical exam within 6 months of start.
Negative TB test or chest X-ray within 10 weeks of start; TB questionnaire annually.
Immunizations or titers required: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
COVID-19 seasonal vaccination/documentation required (exemptions accepted).
Flu seasonal vaccination/documentation required (exemptions accepted; mask required if declined).
Respirator medical clearance annually if respirator use is required.

Dress code and tobacco policy adherence.
BayCare Online Learning Center modules must be completed prior to start (4-8 hours).
Photo for badge required prior to start.
Personal or auto insurance required and must be current for home health positions.
BayCare AHCA privacy policy, affidavit, and demographic information sheet required.
BayCare Fit Mask Acknowledgement required.
BayCare EMR Signature Request Form required for home health candidates.
References: 2 professional references from past 2 years required.
