Registered Nurse Travel/Contract Opportunity
Location: Huntington, West Virginia
Facility Type: Short Term Acute Care, Major Teaching Hospital, Level II Trauma
Total Staffed Beds: 330
Employment Type: Travel/Contract
Contract Length: 13 weeks
Experience Required
- Minimum 2 years of nursing experience
Work Environment & Details
- Scrub Color: Black scrubs (Cabell T-shirt may be worn instead of scrub top)
- Surgery Reporting: Must change into provided surgery scrubs when reporting to surgery
- Charting System: Cerner
- Parking: Free for travelers (lot and garage available)
Required Credentials & Documentation
Identification
- Government issued photo ID (Driver’s License/State ID) required and must be carried during hours of service
- Residency Declaration Form required if working on a compact license
Background Checks
- 7-year criminal background check within 1 year of start, updated annually
- Background check must cover all locations lived/worked in past 7 years
- Background check within 30 days of start required
- International background check if lived/worked abroad
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for contract renewals
Certifications
- ACLS (AHA only) as required
- BLS (AHA only) required (not required for MLT or SPT)
- PALS (AHA only) as required
- NRP as required
- TNCC as required
- State Nursing License primary source verification required
- NURSYS verification required for all RNs
- NIH Stroke Certification required for all RNs and LPN/LVNs except OR RNs (must be completed onsite within 2 weeks if not current)
- CPI (Crisis Prevention Institute) required for ICU staff based on cost center
- Orientation Safety Manual Acknowledgement (Aya HIPAA or equivalent)
Competencies
- Medication Test/Pharmacology Competency Exam as applicable
- Competency Exam required
- Core Parts I, II, III for subcontractors (HIPAA form or core testing acceptable)
Facility-Specific Requirements
- Facility assigns HealthStream modules prior to start; must be completed within 1 week of start
- Orientation and Annual Education review and attestation required before start
- Confidentiality Agreement must be completed prior to start
- Religious and Medical Flu Exemption forms must be submitted and approved prior to start if applicable
- N95 fit test completed onsite at orientation (male candidates must be clean shaven)
- New account creation required for clock-in system prior to start
- COVID declination form required if proof of updated COVID vaccination not provided
- User ID and IT Access Agreement must be completed before start
- Vaccine religious and medical exemption forms must be submitted and approved prior to start if applicable
Health & Immunization Requirements
- Initial physical exam within 1 year of start, updated annually
- Tetanus/Diphtheria/Tdap within 10 years of start
- Negative TB test (PPD, Qgold, or Tspot) within 1 year of start; annual TB questionnaire required
- Measles (Rubeola), Mumps, Rubella: Positive titer or 2 vaccines required; boosters allowed to pend at start per CDC guidelines
- Hepatitis B: Titer, vaccine series, or declination required
- Varicella: Positive titer or 2 vaccines required; boosters allowed to pend at start per CDC guidelines
- Drug Testing: Must follow facility requirements; drug screen within 30 days of start required with specific panel including amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, opiates, oxycodone, phencyclidine, methadone, propoxyphene, fentanyl, nitrites, creatinine
- Seasonal Flu Vaccination required annually (11/1-3/31); declinations not accepted unless approved medical or religious exemption
- COVID-19 Vaccination: Proof required; candidates strongly advised to have most recent boosters starting September 2023; declinations accepted with facility-specific form
History & References
- Proof of 2 clinical references or hospital evaluations
- Performance evaluation required if worked with the system over 1 year
- Employment application/resume required
- Skills checklist updated annually for applicable units
- Internal evaluation required if worked with the system over 2 years
