Job Opportunity: Home Health Physical Therapist (Travel/Contract)

Job Details

Start Date: ASAP
Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
Patient Load: 1:1 ratio; up to 6 patients per day; 30 productivity points standard
Experience Required: Minimum 1 year of skilled home health experience
First-Time Travelers: Accepted
Weekend Requirement: No
On-Call: No
Floating: No floating between units required
Block Schedule: Not accommodated
Locals Accepted: Yes, within 50-75 mile radius
Pending License: Accepted
Guaranteed Hours: Facility may call off up to one shift every two weeks
Transportation: Traveler must have own transportation
Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
Documentation System: EPIC Remote Client (laptop), Rover app (phone), and Oasis documentation required
Oasis Experience: Required

Certifications Required

BCLS (Basic Cardiac Life Support)
Other certifications may be required based on clinical track but BCLS is mandatory for this role

Licensure & Background Requirements

Must have valid state license or compact license accepted
Background check within 45 days of start, covering all locations lived/worked in past 7 years
Additional state-specific background checks and abuse/neglect searches as applicable
Education verification required
Drug screening (10 panel) within 90 days of start
Physical exam within 1 year of start, signed by MD, NP, PA, or DO
Immunizations and health documents required:
- TDAP (within 10 years) or declination
- Negative TB test or chest x-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination (required during flu season)
- COVID vaccine or declination

Additional Requirements

Must complete Providence Health and Services required WorkRamp training modules prior to start (specific to Oregon region and home health specialty)
Fit mask testing required for travel candidates (updated annually)
References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
Employment application covering at least last 5 years required
Skills checklist updated annually as applicable
