Job Opportunity: Registered Nurse - Home Health (Travel)

Hours: 40 hours per week guaranteed (except during holidays)
Schedule:

• 1 schedule: Thursday, Friday, Saturday, Sunday
• 1 schedule: Saturday, Sunday, Monday, Tuesday

Job Description

• Provide Home Health and Hospice care including admissions, death visits, and symptom management in hospice.
• Manage chronic disease patients (adults 18+) with conditions such as CHF, COPD, Diabetes, Cancer, CVA, wound care, and IV administration.
• 1:1 patient ratio.
• Must be able to adapt to changing schedules and environments.
• Physical demands include driving throughout the day, navigating stairs, uneven landscapes, and working in uncontrolled home environments (possible pets, smoke).
• Must be resilient, have strong time management skills, and be socially adept.
• No on-call or call-back requirements.
• Float to other units/facilities may be required based on census but only within clinical skill set and with approval.

Requirements

• Experience: 2 years current Home Health and Hospice experience (higher value taken from 1-2 years stated).
• Recent OASIS experience required.
• Charting experience with Epic within the last 12 months required.
• Certifications: BLS (American Heart Association), valid Wisconsin RN license, valid driver’s license.
• COVID-19 vaccine required unless medical or religious exemption applies.
• Must be able to work independently and "hit the ground running."
• Scrubs: plain colored, well-maintained.

Additional Information

• Travel requirements include driving 50-100 miles per day with 7-7.5 patient visits in a 10-hour shift.
• Orientation will be during day shifts.
• GSA hours are not guaranteed during orientation week.
• Must be flexible and able to document timely and accurately.
