Job Opportunity: MDS Assessor
Location: Brooklyn, New York

Job Summary
Responsible for conducting assessments and coordinating the completion of MDS forms for all residents in compliance with State and Federal regulations and facility policies.
Key Responsibilities
- Obtain necessary information to perform thorough and accurate MDS assessments.
- Review daily census data.
- Prepare interim schedules and significant change assessments for Medicaid and Medicare daily.
- Input Medicaid and Medicare data into American Health Care Systems from MDS booklets.
- Prepare and transmit MDS/PRI files to the Department of Health using designated software tools accurately and on time.
- Pull quality indicator reports from state software as required.
- File MDS booklets in residents’ charts.
- Order supplies for the MDS office.
- Input data for PRI full house, quarterly assessments, and discharges.
- Distribute reports to leadership.
- Collaborate closely with the Quality Improvement Department.
- Perform other related duties as assigned.
Required Skills & Experience
- Minimum 5 years of clinical nursing experience, including at least 2 years performing MDS completion.
- Ability to work in environments with dust, noise, and other typical facility conditions without undue discomfort.
- Physical abilities including hearing, talking, walking, standing, bending, stretching, sitting for long periods, and standing at patient bedside or procedural areas.
- Ability to read, write, and interpret documents effectively.
- Effective communication skills with patients and coworkers.
- May be required to operate and clean equipment, handle heavy weights, and assist in lifting patients.
- Knowledge and training in hazardous waste and materials policies and procedures.
- Understanding of all relevant codes and regulations.
Physical Requirements
- Routine work involves no exposure to blood, body fluids, or tissue.
- Not required to perform or assist in emergency care or first aid.
Education & Licensure
- Required: Bachelor of Science in Nursing (BSN)
- Required: Licensed Registered Nurse (RN) in New York State
