Job Summary

Seeking a motivated and compassionate Physical Therapist to provide home health physical therapy services. You will be part of a dynamic clinical team focused on helping patients return to and maintain their optimum level of physical functioning.

Job Responsibilities

• Provide physical therapy services as prescribed by a physician, following agency policies and federal/state regulations.
• Perform initial and ongoing assessments, including OASIS assessments, to evaluate patient functioning and ability to perform activities of daily living.
• Develop and revise plans of care in consultation with physicians and care team members to help patients achieve realistic mobility goals.
• Report patient reactions to treatment and any changes in condition to the physician and agency representatives.

Required Qualifications

• Valid Physical Therapist license in Florida
• Minimum of 1 year of physical therapy experience within the last year
• Basic Life Support (BLS) certification issued by the American Red Cross or American Heart Association
