Location: Prescott, Arizona
Contract Length: 13 weeks (approximately 3 months)

Job Description
- Conduct social work assessments, including mental health assessments
- Complete Title 36 petitions as needed
- Discharge planning as needed
- Address homelessness, substance abuse, and social determinants of health (SDOH) consultations
- Perform maternal child assessments
- Report to Department of Child Safety (DCS) and Adult Protective Services (APS) as required
- Typical caseload varies between 12-16 patients per day
- Use Cerner EMR system
Schedule
- Typical hours: 8:00 AM – 4:30 PM
- 40 hours per week, five 8-hour shifts
- Some weekend coverage required, approximately every 3-4 weeks
- Occasional flexibility needed to cover Emergency Department shifts from 10:00 AM – 6:30 PM
- No holiday work required
Required Qualifications
- Valid Social Worker license in Arizona
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Minimum of 1 year of social work experience within the last year
Additional Information
- 1-2 week onboarding with Social Work Manager
- Candidates must not have worked full time, part time, or PRN with any related facilities within the past year within a 50-mile radius
- No prior travel experience explicitly required
