Physical Therapist (PT) - Rehabilitation Therapy

Location: Petaluma, California
Employment Type: Contract
Contract Length: 13 weeks


Position Overview

As a Physical Therapist, you will assess and treat patients to help them regain physical function and improve mobility. You will work within a collaborative healthcare team to design and implement individualized treatment plans that promote independence and enhance residents' quality of life.


Key Responsibilities

  • Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
  • Provide physical therapy interventions to improve strength, mobility, and functional abilities.
  • Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications.
  • Collaborate with interdisciplinary team members (occupational therapists, nurses, physicians) to coordinate care and optimize outcomes.
  • Monitor patient progress and adjust treatment plans as needed.
  • Maintain accurate and timely documentation of assessments, progress notes, and treatment plans.
  • Ensure care plans comply with regulatory requirements and best practices in physical therapy.

Qualifications

  • Valid Physical Therapist license in California
  • Minimum of 1 year of experience as a Physical Therapist, including experience within the last year
