Job Description

Seeking compassionate and skilled Physical Therapists to join a rehab team in skilled nursing and post-acute care settings.

Key Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings
• Develop and implement individualized treatment plans to help residents achieve their highest functional potential
• Collaborate with an interdisciplinary team to ensure coordinated care
• Document accurately and timely according to facility and regulatory standards
• Participate in care conferences and contribute to quality improvement efforts

Requirements

• Active California Physical Therapist license (required)
• Strong clinical and communication skills
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented
• Commitment to high-quality, compassionate care
• BLS/BCLS certification

Contract Length

• 13 weeks
